Request for Assisted Preparation Exception



This form must be completed and approved by the Office of Event Services (Maucker Union) and Department of Residence (DOR) Dining Services/UNI Catering when any group intends to prepare their own food with Dining Services' direction and guidance in the commercial kitchen in Maucker Union or Commons Building.

The Center for Multicultural Education (CME), the International Student and Scholars Office (ISSO) in Maucker Union, Maucker Union Meeting Rooms and the Commons Ballroom are the only locations where this service is available.

UNI Catering is the exclusive caterer for Maucker Union and the Commons Building.

No outside food may be brought into the MAU CME, MAU ISSO, MAU Meeting Rooms, or the Commons catering venues for public consumption without permission from DOR Dining Services/UNI Catering.

Using any other caterer without permission or bringing in your own food/beverages is prohibited and could result in the loss of this privilege and/or risk payment to your caterer.



  • Once completed by the sponsoring organization, this form will be submitted electronically to the Office of Event Services (Maucker Union), DOR Dining Services and you will receive a copy of the request.
  • Once you receive your email copy of the completed form, you will need to obtain the remaining approval signatures and turn your form in to DOR Dining Services General Manager for Catering, 010 Redeker Center (Campus Zip: 0252). The completed form should be turned in as far in advance as possible, usually three (3) weeks in advance of the event so that there is plenty of time to organize and plan the event with the DOR Dining Services.  The specific menu and recipes (include number of servings needed) for each menu item in addition to any other documentation requested by DOR Dining Services should be provided with this form.
  • Once approved, DOR Dining/Catering Services will notify Office of Event Services (Maucker Union) and the Organization when approval has been granted.
  • The DOR Dining Services/Catering will keep the original electronic forms on file.

Terms and Conditions

  • This form must be completed a minimum of three (3) weeks prior to the event.
  • Students should follow all instructions given by the DOR Dining Services staff including appropriate dress and sanitation practices as required by the Iowa Food Code prior to and during food preparation and clean-up.
  • A provided "Wavier of Liability" form must be signed by all students who assist in the preparation of the food in DOR operated facilities. This form will be provided by DOR Catering at the time of the event.
  • The number of students needed for food preparation will be determined by the DOR Dining Services staff. If the recommended number of students not available for food preparation, the event may be cancelled. Students should stay until DOR Dining Services' staff approves the work is complete.
  • All costs associated with event are the responsibility of the sponsoring organization/department. This includes menu ingredients, use of kitchen and serving equipment, professional staff time and any other any other requirements deemed necessary by DOR Dining Services' staff.
  • Future permission to engage in this type of activity is contingent upon compliance with these terms and conditions.


Description of Menu Items

Describe completely all food and beverages to be served/prepared at your event and how they will be prepared.  Vague or incomplete descriptions will delay approval of this form.

You will need to provide a copy of the recipe to the DOR Dining Services Manager.
