Request for Catering Exception

 


This form must be completed and approved by the Office of Event Services (Maucker Union) and Department of Residence (DOR) Dining Services/UNI Catering when any group proposes to use a non-DOR Dining Services or UNI Catering provider for an event on university property. Before finalizing any arrangements for an event, the group should review the specific food policy of the on-campus venue where the event is to be held. UNI Catering is the exclusive caterer for the Maucker Union (MAU), Commons Building, Department of Residence Buildings (DOR) and Gallagher Bluedorn Performing Arts Center (GBPAC). No outside food may be brought into the MAU CME, MAU ISSO, MAU Meeting Rooms, GBPAC, DOR Buildings or the Commons catering venues for public consumption without prior permission from DOR Dining Services/UNI Catering. Using any other caterer without permission or bringing in your own food/beverages is prohibited and could result in the loss of this privilege and/or risk payment to your caterer.

Instructions:

  • Once completed by the sponsoring organization, this form will be submitted electronically to the Maucker Union Administrative Office, DOR Dining Services and you will receive a copy of the request.
  • The recognized student organization/university department must obtain the non-university licensed food service operator’s signature on the Request for Catering Exception Form and then send the completed form to the DOR Dining Services Catering Manager, 010 Redeker Center (Campus Zip: 0252) to obtain the necessary approval signatures.
  • The sponsor must also obtain proof that the operator possesses a current food license and meets University liability insurance requirements. A copy of the food service operators’ certificate of liability insurance and current food license should be sent along with this completed form and returned to the DOR Dining Services Catering Manager, 010 Redeker Center (Campus Zip: 0252) to obtain the necessary approval signatures.
  • The registered student organization/university department will not be allowed to re-sell donated product or sell food from an outside caterer unless the event is part of a larger campus celebration such as Homecoming or Family Weekend, as examples. Once approved, events of this nature will require a temporary food license from the Black Hawk County Health Department.
  • The DOR Dining Services Office will keep the original Request for Catering Exception Form on file and will keep all insurance certificates and other documentation regarding the non-university licensed food service operator.
  • Once approved, DOR Dining/Catering Services will notify Office of Event Services (Maucker Union) and the Organization when approval has been granted.

Terms and Conditions

  • This form must be completed a minimum of two (2) weeks prior to the event.
  • The Food Service Operator is granted limited permission for the distribution of food. This permission may be terminated or cancelled by the MU Administrative Office or DOR Dining Services at any time, at its sole discretion, without recourse by the operator.
  • The Food Service Operator must comply with all directives of University personnel.
  • The Food Service Operator is solely responsible for the management, preparation, and distribution of its food and beverage (including set-up, service, supervision, tear-down, clean-up) as well as all financial obligations and liabilities in connection with or necessitated by this exception permission.
  • The Food Service Operator is responsible for providing any and all equipment and personnel necessary to conduct its business.
  • The Food Service Operator is responsible for obtaining all required licenses or permits and fully complying with applicable federal, state, local laws and regulations as well as all pertinent University policies.
  • The Food Service Operator shall defend, indemnify and hold harmless the University, and its officers, agents, and employees, from and against any and all claims, damages, costs, losses, liabilities, or expenses, including reasonable attorneys’ fees, arising out of or in any way connected with this exception including, without limitation, claims for any loss or damage to property, or for death or injury to any person or persons.
  • The Food Service Operator, at its sole cost and expense, must insure its activities; and provide the University with a certificate of insurance.
  • The Food Service Operator may not assign or delegate this permit to any third party without specific written permission of the MU Administrative Office or DOR Dining Services.
  • This permit is governed by and interpreted under the laws of the State of Iowa.
  • Future permission to engage in similar events is contingent upon compliance with these terms and conditions.
